[Abstract] Introduction and objectives. To assess the prognostic value of a 5-item clinical model based in the IC-BERG recommendations to evaluate ambulatory patients with heart failure (HF) in the clinic. Methods. Observational study based on the historical cohort of patients with HF referred to a specific facility since 2010 to 2019. The presence of 5 clinical ítems was evaluated: NYHA III-IV class, signs of congestion, admission due to decompensation in the previous year, daily dose ≥ 40 mg furosemide or equivalent, and NT-proBNP ≥ 1,000 pg/ml. The incidence of adverse clinical events was assessed by means of the Kaplan-Meier method and multivariable Cox's regression. Results. We studied 1909 patients, whose mean number of clinical ítems indicating risk was 2.29. Kaplan-Meier survival analysis showed an incremental trend between an increasing number of clinical ítems of risk and the combined event death or admission due to HF, overall mortality, and cardiovascular mortality (P < .001). Adjusted hazard-ratio for the combined end-point death or admission due to HF, as estimated by means of multivariable Cox's regression, were 1.47 (95%CI, 1–2.15), 2.03 (95%CI, 1.41–2.92), 2.98 (95%CI, 2.08–4.28), 5.07 (95%C,: 3.51–7.32), and 7.73 (95%CI, 5.21–11.45) for patients who showed 1, 2, 3, 4, or 5 clinical ítems indicating risk, respectively. The proportional weight of cardiovascular causes of risk, especially refractory HF, was higher in patients with a higher number of clinical ítems of risk. Conclusions. [Abstract] Faced with the impossibility of attending in person to carry out certain clinical practices, due to the vulnerability of its users to COVID-19, the Faculty of Physiotherapy has decided that certain blocks of content are taught in person in the designated laboratories for this purpose in the Faculty itself. Faced with this situation, the idea arises of using videos of real patients from the center to which we cannot attend in a present way, to be used as a support didactic resource in the teaching of Clinical Stay II (4th year). After obtaining the consent of the clinical center, 32 clinical cases are recorded, in videos of 12-15 minutes, with the aim of being used as a support didactic resource in the teaching of the Clinical Stay II of the 4th year, during the entire 2nd semester (February-May 2021; 14 weeks), to work with the students on the process of "clinical reasoning", in addition to trying to bring them closer to a clinical reality....

Eficacia del manejo clínico alternativo del periné para reducir el traumatismo perineal intraparto

[Resumen] Una constante en la práctica enfermera obstétrica es evitar los efectos indeseados del traumatismo perineal en el parto y aspirar a un periné lo más íntegro posible, que asegure una recuperación postparto de calidad y con las mínimas secuelas físicas y psíquicas para la mujer. Sin embargo, no existe una evidencia científica avalada en la práctica que asegure que determinadas posturas adoptadas por las parturientas o determinadas maniobras de protección sean mejores que otras. La mayoría de las matronas y obstetras eligen una maniobra de protección u otra en virtud de su formación, experiencia profesional, convicciones personales o según la rutina habitual de cada centro sanitario, siendo más una práctica intuitiva que una práctica clínica contrastada científicamente.
Sustituir la arbitrariedad en los cuidados enfermeros y adoptar protocolos y guías clínicas de buenas prácticas (BBPP) en todas las salas de dilatación y parto, que estén avaladas por la evidencia científica, es una emergerte necesidad según la filosofía de los cuidados de Enfermería Basada en la Evidencia (EBE).
Objetivo principal: Medir la eficacia de una intervención de protección del periné alternativa a la maniobra habitual para reducir las lesiones perineales intraparto en las mujeres que son asistidas en el parto en el Hospital Arquitecto Marcide de Ferrol.
Metodología: Estudio experimental, ensayo clínico aleatorizado, abierto, simple y controlado, desarrollado en la sala de dilatación y partos del Hospital Arquitecto Marcide desde enero de 2020 a enero de 2022, sobre una muestra de 210 mujeres en la segunda etapa del parto.
Se diseña un ECA con dos grupos de intervención independientes, uno de ellos con una postura de Sims, pujos libres, aplicación de compresas calientes y protección “Hands Off” y otro grupo en el que se adopta la postura clásica de litotomía, pujos dirigidos inmediatos y protección manual activa o “Hands On”....

[Abstract] A constant in obstetric nursing practice is to avoid the undesirable effects of perineal trauma during childbirth and to aspire to a perineum that is as complete as possible, ensuring a quality postpartum recovery with the minimum physical and psychological consequences for the woman. However, there is no scientific evidence backed up in practice to ensure that certain postures adopted by women giving birth or certain protective manoeuvres are better than others. The majority of midwives and obstetricians choose one protection manoeuvre or another by virtue of their training, professional experience, personal convictions or according to the usual routine of each health centre, being more an intuitive practice than a scientifically contrasted clinical practice. Replacing arbitrariness in nursing care and adopting protocols and guidelines for best practices in all dilation and delivery rooms, which are supported by scientific evidence, is an emerging need according to the philosophy of Evidence-based Practice. Objective: To measure the efficacy of a perineal protection intervention alternative to the usual maneuver to reduce intrapartum perineal lesions in women who are assisted in childbirth at the Hospital Arquitecto Marcide in Ferrol. Methodology: Experimental study, randomized clinical trial, open, simple and controlled, developed in the dilation and delivery room of the Hospital Arquitecto Marcide from January 2020 to January 2022, on a sample of 210 women in the second phase of labour. [Abstract] Introduction: Scheuermann’s Disease (SD) or juvenile dorsal kyphosis is an alteration of the vertebral spine (VS) in the sagittal plane that presents specific radiographic characteristics and for which there is no consensus about the effectiveness of physiotherapy treatment and the treatment of choice. Objective/Purpose: to identify the available evidence about case reports addressing patients with this pathology; and to determine the effectiveness of a physiotherapy intervention based on therapeutic exercise (TE) on the symptomatology, physical dysfunctions, and participation on a patient with SD. Furthermore, compare the patient’s satisfaction according to the modality of the sessions (face-to-face vs. telerehabilitation vs. autonomous sessions). Methods and material: literature review in the Medline database of case studies about SD published between 2000 and the present; and a case report of a 22-year-old patient diagnosed with SD with associated recurrent acute pain and atypical characteristics of the disease, in which physiotherapy treatment is applied based on a therapeutic program is applied 3 days a week for 6 weeks, and pre- and post-intervention results are compared. The program includes exercises of respiratory pattern re-education, motor control, flexibility, neural mobilization, and muscle re-education. Results/Outcomes: 19 articles were included, of which only 2 of them applied physiotherapy treatment, based on the Schroth method, proving to be effective. The results of the intervention performed showed improvements in terms of short-term symptomatology reduction, maintenance of the level of participation, reduction of lumbar hyperlodosis, greater global dorsolumbar mobility, improvement of muscle function, and greater motor control of the lumbopelvic region. The patient reported the higher level of satisfaction with the directed face-to-face sessions modality. Conclusions: there is very little evidence on the effectiveness of physiotherapy in patients with SD when analysed through case study methodology, although the results are positive. A clinical case study based on a young patient with SD has shown that a physiotherapy intervention is effective in the short term in terms of symptomatology, morphology, function and participation; and that satisfaction with the intervention is higher with face-to-face sessions....
