Abordaje de la parálisis facial desde la fisioterapia: revisión sistemática

[Abstract] Backgrounds: Facial paralysis is a common pathological entity, of varied etiology and evolution, which causes physical, functional, aesthetic, and psychosocial deficits. Clinically, paralysis of all the upper and lower facial muscles (peripheral facial palsy) or paralysis that only affects the lower portion of the face (central facial palsy) can be seen. Currently, there is no consensus on the treatment that should be prescribed in facial paralysis, with pharmacological treatment being the treatment of first choice. Objective: To identify which are the techniques, resources and/or therapeutic modalities used from physiotherapy in the approach to facial paralysis. Material and method: A systematic bibliographic review was conducted during the months of March-April 2022, consulting 7 databases: Cochrane, Pubmed, Dialnet, Web of Science (WOS), CINAHL, PEDro, and Scopus. Meta-analyses, systematic reviews, reviews, controlled and/or randomized clinical trials, and clinical trials were included, published between January 2017 and April 2022, published in English or Spanish, in which physiotherapy techniques, resources or therapeutic modalities are used to address the symptoms present in patients of any age with facial paralysis, where the evaluation and/or intervention used from physiotherapy is described. Outcomes: Finally, 26 articles were selected that met the inclusion criteria defined for this search (13 systematic reviews, 12 clinical trials and 1 quasi-experimental study). The physiotherapy resources identified for the approach to facial paralysis were motor imagery, mirror therapy, mimotherapy, neuromuscular re-education, proprioceptive neuromuscular facilitation, electrotherapy, low-intensity laser, wet cupping, acupuncture, tape feedback, vibration, facial expression exercises and orofacial exercises, orofacial therapy based on the Castillo Morales method, surface electromyography feedback, stretching, massage, and hot packs. Conclusions: The bibliography shows that physiotherapy achieves significant improvements in the symptoms present in facial paralysis, however, the heterogeneity of the studies and the lack of evaluation and treatment protocols prevent definitive conclusions about the effectiveness of the therapeutic resources used from physiotherapy....
[Resumen] Antecedentes: A parálise facial é unha entidade patolóxica bastante frecuente, de etioloxía e evolución variada, que provoca déficits físicos, funcionais, estéticos e psicosociais. Clinicamente pódese observar parálise de tódolos músculos faciais superiores e inferiores (parálise facial periférica) ou parálise que só afecta a parte inferior da cara (parálise facial central). Actualmente, non existe consenso sobre o tratamento que se debe prescribir na parálise facial, sendo o tratamento farmacolóxico o tratamento de primeira elección. Obxectivo: Identificar cales son as técnicas, recursos e/ou modalidades terapéuticas empregadas dende a fisioterapia na abordaxe da parálise facial. Material e método: Realizouse unha revisión bibliográfica sistemática durante os meses de marzo-abril de 2022, consultando 7 bases de datos: Cochrane, Pubmed, Dialnet, Web of Science (WOS), CINAHL, PEDro, e Scopus. Incluíronse metaanálises, revisións sistemáticas, revisións, ensaios clínicos controlados e/ou aleatorizados, e ensaios clínicos, publicados entre xaneiro de 2017 e abril de 2022, publicados en inglés ou español, nos que se utilizan técnicas, recursos ou modalidades terapéuticas de fisioterapia para abordar os síntomas presentes en pacientes de calquera idade con parálise facial, onde se describe a avaliación e/ou intervención empregada dende a fisioterapia. Resultados: Finalmente, seleccionáronse 26 artigos que cumpriron os criterios de inclusión definidos para esta busca (13 revisións sistemáticas, 12 ensaios clínicos e 1 estudo case-experimental). Os recursos de fisioterapia identificados para o abordaxe da parálise facial foron a imaxinería motora, a terapia de espello, a mimoterapia, a reeducación neuromuscular, a facilitación neuromuscular propioceptiva, a electroterapia, o láser de baixa intensidade, as ventosas húmidas, a acupuntura, a retroalimentación con cinta adhesiva, a vibración, os exercicios de expresión facial e exercicios orofaciais, a terapia orofacial baseada no método Castillo Morales, a retroalimentación mediante electromiografía superficial, estiramentos, masaxes e compresas quentes. Conclusións: A bibliografía mostra que a fisioterapia acada melloras significativas nos síntomas presentes na parálise facial, non obstante, a heteroxeneidade dos estudos e a falta de protocolos de avaliación e tratamento impiden chegar a conclusións definitivas sobre a eficacia dos recursos terapéuticos empregados dende a fisioterapia....

[Abstract] Objective. Make an analysis of existing scientific evidence about the efficacy of extracorporeal shock waves therapy in fasciitis plantar treatment. Methods. For preparing this bibliographic review it has been performed a search of the scientific literature published in PubMed, Scopus, PEDro, CINAHL, SportDiscus y Cochrane, between February and May of 2016. The selection of the articles was made based on the objective of the work, limiting results to only clinical trials. Likewise, it has been limited by year of publication, from 2011 until now, and related to language, dismissing those written in a language different from English, Spanish or Portuguese Outcomes. After application of exclusion and inclusion criteria there has been selected a total of 20 studies of which 10 are randomized controlled clinical trials, 8 clinical trials, 1 not randomized controlled clinical trial and 1 randomized uncontrolled clinical trial. Its analysis was performed focusing in the study type, objective, sample size, shock waves configuration parameters, methodology, measuring system and obtained results. ESWT in chronic FP treatment show improvement in the results of VAS, RMS, and AOFAS scales, as well as a reduction in the thickening of the plantar fascia. The best results were obtained by combining shock waves therapy with a specific stretch of the plantar fascia and sural triceps. Conclusions. There is significative scientific evidence about the efficacy of extracorporeal shockwave therapy in chronical fasciitis plantar treatment. There have been successful results at short, middle and long term as to pain reduction, better functionality and reduction in the thickening of the plantar fascia. Its use in this pathology seems effective...
