Abstract
[Resumen] Antecedentes. El dolor lumbar (DL) es la principal causa mundial de años vividos con discapacidad,
y la investigación sobre inteligencia artificial (IA) aplicada a su manejo ha crecido de forma
acelerada y fragmentada, sin una lectura integrada de su estructura. Este Trabajo de Fin de Grado
mapea y caracteriza dicho campo mediante una revisión híbrida bibliométrica y computacional de la
literatura.
Material y métodos. Se interrogaron cinco bases de datos (PubMed, Scopus, Web of Science, IEEE
Xplore y CINAHL) para el período 2016–2026. Tras la deduplicación en cascada y un cribado de
relevancia asistido por un modelo de lenguaje —validado frente a un gold standard humano (κ =
0,960)—, se retuvieron 677 registros, sobre los que se realizó un análisis bibliométrico e
interdisciplinar y un modelado de tópicos con BERTopic (13 tópicos). Una optimización submodular
seleccionó 34 estudios representativos, extraídos mediante doble LLM independiente (Claude Opus 4.8
y GPT-5.5) con adjudicación humana.
Resultados. La producción procede de 70 países —liderada por Estados Unidos y China—, crece hasta
2025 y se organiza en dos macrorregiones (imagen y aprendizaje profundo frente a clínico-conductual
y de señales), con un marcado carácter interdisciplinar (el 96,7 % de los documentos abarca dos o
más subcampos). Los 34 estudios se concentran en el diagnóstico (23 de 34) y en el aprendizaje
profundo y clásico, pero muestran una baja madurez traslacional (19 en prueba de concepto; solo 2
con despliegue clínico) y una brecha de reproducibilidad (la explicabilidad no se reporta en 19 de
34). La extracción dual fue fiable (precisión del 85,0–87,5 % frente al
gold standard).
Conclusiones. El campo, en expansión pero metodológicamente inmaduro, exige mayor validación
externa, explicabilidad y transparencia; la arquitectura propuesta aporta una síntesis reproducible
y auditable de un dominio interdisciplinar en rápida
evolución.

This Bachelor’s thesis maps and characterizes this field through a hybrid bibliometric and computational literature review. Methods. Five databases (PubMed, Scopus, Web of Science, IEEE Xplore and CINAHL) were searched for the 2016–2026 period. After cascade deduplication and a large language model–assisted relevance screening —validated against a human gold standard (κ = 0.960)—, 677 records were retained and analysed bibliometrically and across disciplines, with BERTopic topic modeling (13 topics). A submodular optimization selected 34 representative studies, extracted via dual independent LLMs (Claude Opus 4.8 and GPT-5.5) with human adjudication. Results. Output originates from 70 countries —led by the United States and China—, grows until 2025, and is organized into two macro-regions (imaging and deep learning versus clinical-behavioral and signal-based), with a markedly interdisciplinary nature (96.7 % of documents span two or more subfields). The 34 studies concentrate on diagnosis (23 of 34) and on deep and classical learning, but show low translational maturity (19 at proof-of-concept; only 2 with clinical deployment) and a reproducibility gap (explainability unreported in 19 of 34). Dual extraction was reliable (85.0–87.5 % accuracy against the gold standard). Conclusions. The field, expanding yet methodologically immature, calls for greater external validation, explainability and transparency; the proposed architecture provides a reproducible and auditable synthesis of a rapidly evolving interdisciplinary domain.
