[Abstract] Cataracts surgery with an IOL is one of the most common medical interventions in the world,
with more than 20 million procedures done yearly. Even though it is a very common procedure,
there are still complications associated with the IOLs, like the opacification of the
lens and the phenomenon known as glistening. Glistening is characterized by the presence of
small vacuoles of liquid inside the lens, which can generate symptoms such as a loss of vision,
halos, glare or loss of sensibility to contrast in some cases.
In this study we present a deep learning and image processing based methodology to
better the accuracy and speed of diagnostics by segmenting and classifying glistening on ASOCT
images. To do this we follow three approaches, first we train a classification network to
try and classify the images directly. Secondly we first train a segmentation model to segment
the full image and then classify the images based on the results of said segmentation, and
thirdly we first train a segmentation model to segment the region of interest from the image,
then from those results we train another segmentation network to segment the glistenings
and lastly we classify the images depending on the results from that last step. For the first
approach we tested two architectures, DenseNet and ResNet, and for the second and third
approaches we tested U-Net, PAN, PSPNet and DeepLabV3.
This is a novel study researching the multiclass classification of glistenings with the use
of deep learning, utilizing images provided to us by specialists in the field and which results
will be used by said specialist to help with the diagnosis process.
This project can have a big impact in the medical world due to the amount of people that
suffer from complication from cataracts surgery. A more accurate and earlier diagnosis could
improve their quality of life by a big margin. [Resumo] A cirurxía de cataratas con LIO é unha das intervencións médicas máis comúns no mundo,
con máis de 20 millóns de procedementos realizados anualmente. A pesar de ser un procedemento
moi común, aínda existen complicacións asociadas ás LIOs, como a opacificación do
cristalino e o fenómeno coñecido como glistening. O glistening caracterízase pola presenza de
pequenas vacuolas de líquido no interior do cristalino, que poden xerar síntomas como perda
de visión, halos, cegamentos ou perda de sensibilidade ao contraste nalgúns casos.
Neste estudo presentamos unha metodoloxía baseada en aprendizaxe profunda e procesamento
de imaxes para mellorar a precisión e rapidez dos diagnósticos segmentando e clasificando
o glistening en imaxes AS-OCT. Para iso seguimos tres enfoques, en primeiro lugar
adestramos unha rede de clasificación para tentar clasificar as imaxes directamente. En segundo
lugar, primeiro adestramos un modelo de segmentación para segmentar a imaxe completa
e logo clasificamos as imaxes en función dos resultados da devandita segmentación, e
en terceiro lugar, primeiro adestramos un modelo de segmentación para segmentar a rexión
de interese da imaxe, logo, a partir deses resultados, adestramos outra rede de segmentación
para segmentar os brillos e, por último, clasificamos as imaxes en función dos resultados dese
último paso. Para o primeiro enfoque probamos dúas arquitecturas, DenseNet e ResNet, e o
segundo e terceiro enfoque probamos U-Net, PAN, PSPNet e DeepLabV3.
Este é un estudo novo que investiga a clasificación multiclase de glistenings co uso de
aprendizaxe profunda, empregando imaxes proporcionadas por especialistas no campo e cuxos
resultados serán empregados polos devanditos especialistas para axudar co proceso de
diagnóstico.
Este proxecto pode ter un gran impacto no mundo da medicina debido á cantidade de
persoas que sofren complicacións derivadas da cirurxía de cataratas. Un diagnóstico máis
preciso e precoz podería mellorar a súa calidade de vida en gran medida.
